Acoustic Radiation Force Impulse (ARFI) imaging employs sound impulses to induce and track tissue motion that conveys mechanical property. In application to carotid atherosclerotic plaque, ARFI noninvasively delineates features that convey risk for ischemic stroke: intraplaque hemorrhage, lipid-rich necrotic core, thin or ruptured fibrous cap, diffuse collagen, and calcium deposits. This talk will feature recent advances to in vivo ARFI atherosclerosis imaging technology, including the use of machine learning for automated feature classification, which are validated histologically in patients undergoing carotid endarterectomy. New developments, including incorporating vector blood flow estimation for combined shear strain assessment, will be highlighted.
